The Family Supports for Children with Disabilities (FSCD) program and the Persons with Developmental Disabilities (PDD) program work with individuals, children, and families within the differing program’s mandates and legislation, Family Support for Children with Disabilities Act and the Persons with Developmental Disabilities Services Act. FSCD/PDD work to understand the support needs of individuals, children and families to provide individualized supports and services within the scope of each program.

In the Caseworker role, you will be responsible for working collaboratively with children, families, individuals and guardians in identifying, and assessing support needs that are within the Family Support for Children with Disability (FSCD)/ Person with Developmental Disabilities (PDD) program mandate, in order to assist in supporting children and adults with disabilities. Activities include assessment, referrals, service planning, commitment to supports and services, and ongoing monitoring and support. 

 

Duties include, but are not limited to:

  • Transition Planning: working with individuals, families and other GoA programs to support smooth transitions for youth who are receiving supports as a child, and who may be eligible for PDD- funded services as adults. 
  • Service Design and Planning based on the supports required and monitoring (ongoing case management), planning and review of an individual's progress in programs and services, including evaluation of the appropriateness of the services provided.
  • Family Managed Services (FMS): clarify the role of the Funds Administrator and monitor their responsibilities in relation to the FMS agreement (providing information related to planning and delivering services to the Individual, monitoring services, hiring and monitoring staff, and reporting requirements); assist the Funds Administrator in planning services for the Individual.
  • Partnering with families who have children with disabilities, and providing proactive supports and services to these families, which enhance their capacity to promote their child's healthy growth and development.
  • Using knowledge of family centered practice to work collaboratively with parents to complete ongoing assessments of the child and family's needs, which will result in an Individualized Family Support Plan that will result in an FSCD agreement. The family is a partner with you in this relationship and will have opportunities to participate in all aspects of the process. 
  • Helping the family coordinate supports and services to address new or changing needs of the child with the disability, as well as the needs of the family.
  • Monitoring the delivery of supports and services and review progress toward the achievement of outcomes.
  • Establishing, cultivating and enhancing working relationships with community partners to maximize support and services to families. 

Qualifications

Minimum Recruitment Standard: 

A Bachelor's degree in Social Work with some related experience. 

 

Equivalencies considered: 
Related Education: Master of Social Work (no experience required); or related degrees in Social Sciences (including psychology, community development, rehabilitation or disability studies) with a minimum of two years of related experience; or related diploma and 3 years related experience; or related certificate and 4 years related experience. 

 

Please note: Successful applicants who possess a Bachelor of Social Work, Master of Social Work  or Diploma in Social Work from an Alberta College, will be required to register and maintain registration with the Alberta College of Social Workers. 

 

Related Experience: case management, planning and/or arranging services and supports for individual’s with disabilities, and/or children, and/or other vulnerable individuals. Experience with service plans, community based resources, and managing a caseload. 

 

A valid Class 5 operator's license and access to a reliable vehicle is essential, as travel is a requirement of the role. 

 

Assets: 

  • Experience working with persons with disabilities to promote their independence and community inclusion. 
  • Knowledge of a broad range of topics related to the disability field, relevant resources and case management practices.
  • Experience working collaboratively with families, community partners and other government programs.
  • Experience with interpreting and applying government legislation, regulations and policy/procedures in practice. 
  • Ability to clearly communicate rationale for decisions.
  • Awareness of local community resources and experience providing referral information.

Salary

$2,589.62 to $3,347.57 bi-weekly ($67,589 – $87,371/year)

Notes

  • Hours of work: Full Time: 36.25 hrs/week; Monday to Friday work week.
  • The purpose of this competition is to create a Talent Pool for full time permanent and/or temporary positions, located throughout the North Zone of AB.  The North Zone includes Edmonton, Grande Prairie, Fort McMurray, Slave Lake, Peace River, Westlock, St. Paul, Edson, Hinton, Whitecourt and communities in between. 
  • Candidates have to be based out of Alberta and willing to relocate 
  • Candidates with less qualifications may be considered and remunerated at a lower classification.
  • Travel is a requirement for the position and incumbents must have a valid driver’s license and access to a reliable vehicle.
  • Final candidates for this competition will be asked to undergo an academic verification and security screening. Due to wait times to receive a completed Criminal Records Check (CRC) that includes Vulnerable Sector screening check, applicants that are invited for an interview are encouraged to request a completed CRC (level 1) that includes Vulnerable Sector screening check from their local RCMP/Police detachment prior to their interview. The CRC will only need to be provided upon request. Additionally, a current and clear Intervention Records Check will need to be provided upon request.
  • This position may be eligible for northern retention allowance, depending on where in the North Zone the candidate resides.
