Within the Ministry of Children and Family Services there are four Provincial Campus Based Care (PCBC) that provide 24-7 campus-based care to adolescents with significant trauma-related issues. Programs include both Intensive Treatment and Secure Services in the following facilities:
 

  • Yellowhead Youth Center - Provincial Campus Based Care (PCBC)
     

Role Responsibilities

The Transitions and Connections Specialist will support the alignment of Provincial Campus Based Care (PCBC) practice. Primary activities are to work alongside the PCBC Multi-Disciplinary Team (MDT) to ensure that youth's care plan supports the safety of the youth and integrity of the family. The Transitions and Connections Specialist provides short-term involvement to assist youth from the Intensive Care programs in the successful integration to their next placement. This program offers a bridge to youth so that support services can be arranged, and every attempt is geared towards a successful return to the community. The goal of the program is to have the youth leaving the PCBC with an established community of connection and support that is meaningful for them. This includes connections to the community, cultures, heritage, and spirituality.

As a Transitions and Connections Specialist, some of your responsibilities include: 

  • Participate in the multi-disciplinary treatment team, providing input on family issues, recommending resources, and adjusting treatment strategies to support youth transitions back to family or placement resources.
  • Prepare and documents detailed records of family and community contact for presentation to the treatment team and professionals.
  • Oversee family involvement in the residential program, providing orientation at intake and support throughout treatment, discharge, and post-discharge phases.
  • Develop home visitation plans, safety contracts, and relationships with youth and families to support the treatment process.
  • Provides crisis intervention, problem resolution, and provide support for youth and parents on topics like anger management, grief, and relationship building.
  • Offer client-specific consultation to caregivers, community agencies, and service providers to assist with care plans and family systems issues.
  • Liaise between community partners and families.
  • Ensure Ministry policies and procedures are implemented; to support critical thinking and analysis, and to ensure that program services comply with legislation, regulations and ministry policy and procedure.


To be successful in this position, you must demonstrate strong leadership, and critical thinking skills. You will have related theoretical knowledge of childcare practice; assessment, treatment, and behavioral management, as well you will draw upon your exceptional written and verbal communication skills to be effective relationships with others.

Qualifications

Related diploma or related degree plus three years progressively responsible related experience.
 

Requirements:

  • Plus, considerable progressively responsible experience in a campus-based treatment setting with 3-5 years of experience as a Child & Youth Care Worker 2 or 3-5 years of other supervisory experience in a related field.
  • Registration with the Alberta College of Social Workers (ACSW) is required for all social work graduates.
  • Certification with the Child and Youth Care Association of Alberta is preferred.
  • You must have a valid Class 5 Driver’s License

  • Child and Youth Care Degree or Diploma (or related field) preferred.
  • Significant knowledge supplemented by experience is required of theoretical models, specialized techniques, and practical and theoretical procedures related to assessment, intervention, dispute resolution, family violence and court processes/procedures is an asset.
  • Current Certification in First Aid with CPR and Suicide Intervention.

Location:   Edmonton, Alberta (flexibility to travel to all Provincial Campus Based Care (PCBC) locations.
